4. Name of the wearer (& relationship to baby)
5. Brand and type of carrier used (e.g. DIY pouch)
6. Email address
7. Contact Number
8. Date photo was taken

Terms & Conditions:

1. Open to Malaysians and residents of Malaysia. Malaysians staying abroad are welcomed to participate but are to bear postage costs of prizes if appears to be the resulting winner.
2. Proof of postage is not proof of receipt.
3. Submitted entries becomes the property of Malaysian Babywearers and are non-returnable.
4. All entries are to reach us no later than 25th October 2009.
5. Results will be posted online (in our blog) on 9th November 2009.
6. Winners will be notified via email. Please ensure email is provided and correct.
7. Results are final and no correspondence will be entertained.
8. Malaysian Babywearers reserves the right to ammend the terms and conditions without prior notice.
